- Tracks and assesses a patients’ medical status in a timely and ongoing basis and ensures NO medical issues fall through the cracks

Ensures there is NO negation of the Primary Care Physician’s OHIP fees for the provisioning of OHIP services at any time during the course of medical management.
Why we know this is important: your practice is busy, and we understand that you are unable to follow up with all your clients on a monthly basis and as a result, your clients may not be attending the appropriate medical treatment and documenting their injuries and limitations as required to ensure that their claim is maximized.
During our monthly telemedicine appointments, our Licenced Physicians will review your clients’ needs which will ensure your clients’ injuries are well documented and their medical needs are addressed.
Program Outline
Patients receive 1~2 OHIP covered Telemedicine Consultations per month by Licensed Canadian Physicians (note. frequency of consultations is dependent upon the severity of the medical presentation)
Doctor-Patient Telemedicine Consultations result in the production of an OHIP consultation note which addresses the patient’s medical status in real-time and results in the diagnosis and treatment of acute and chronic medical issues.
Licensed Canadian Physicians refer patients to Medical Specialists (i.e., Chronic Pain Specialists, Psychiatrists, Neurologists, Orthopaedic Surgeons, etc.), Allied Healthcare Services, and for Diagnostic Investigations. Patients are assessed by Conciergedoc’s network of Specialists in a timely fashion.
Currently, greater than 50% of Conciergedoc’s patient population are being assessed by Psychiatrists on a weekly basis. The turnaround time to scheduling appointments with our network of Psychiatrists is 1~2 weeks.
Clinical Notes and Records (CNR’s) are collected, organized chronologically, and included in a Health Status Monitoring Report which is updated as new consultation notes are provided by each medical specialist on an ongoing basis.
Licensed Canadian Physicians complete standard Insurance forms such as OCF-3’s, OCF-18 (Treatment Plans) and OCF-19’s as required, as well as Federal and Provincial Disability Program applications
CONCIERGEDOC ensures there is NO negation of the Primary Care Physician's OHIP fees for the provisioning of OHIP services at any time during the course of medical management. CONCIERGEDOC complements the Primary Care Physicians medical management of the patient by way of ongoing collaboration, co-ordination, communication and consultation.

Request to enrol clients by emailing enrol@conciergedoc.ca. CONCIERGEDOC will contact you to review all pertinent details and to confirm the enrollment.
Schedule
CONCIERGEDOC proceeds to contact and schedule your clients’ telemedicine consultation after which you will be notified.
Track
CONCIERGEDOC provides the referral source a Health Status Tracking Report which is updated on an ongoing basis and includes all OHIP consultation notes.
