  • Any Licensed Physician in Canada can refer patients.
Who can refer a client?
  • Lawyers, Paralegals, Rehabilitation Centres, Case Management Services, and Medical Assessment Centre’s. Anyone can refer clients who have suffered traumas due to personal injuries.
What type of patient population are Conciergedoc physicians’ experts in managing?

Patients who have suffered traumas due to personal injuries such as:

Conciergedoc’s network of OHIP covered Family Physicians and Medical Specialists enables patients to be evaluated in a timely fashion.

What are the benefits to Lawyers, Paralegals, Rehabilitation Centres, Case Management Services, and Medical Assessment Centre’s in enrolling clients in the Health Status Monitoring Program?
  • Enrolment guarantees a client will be assessed 1~2 times per month by a Family Medicine Physician via OHIP.  This detail alone provides the referral source a sense of calmness knowing their client will be assessed routinely
  • All telemedicine consultations result in an OHIP consultation note which documents the medical status of the patient on an ongoing basis
  • Family Medicine Telemedicine Consultations subsequently result in referrals to Medical Specialists. These Medical Specialists include but are not limited to:
  • Conciergedoc Telemedicine Physicians complete Insurance Forms. These forms include:
    • OCF-3’s
    • OCF-18 Treatment Plans
    • OCF-19’s
  • Conciergedoc Telemedicine Physicians schedule Diagnostic Imaging Studies in a timely fashion
  • Conciergedoc Telemedicine Physicians schedule appointments with Allied Health Professionals

Our team of Physician’s focuses on Patient Care and providing a Case Management approach by directly communicating with the patient’s representative to ensure Maximum Medical Recovery is achieved and to ensure NO Medical Issues fall through the cracks.

 

Our program focuses on managing the medical issues which have resulted secondary to the personal injuries.  All OHIP consultation notes are provided to the referring physician on an ongoing basis. 

Is there a fee to enrol clients into the Health Status Monitoring Program?

All Telemedicine Consultations performed in Ontario by Licensed Canadian physicians are covered by OHIP. 

CONCIERGEDOC bills a nominal fee for patients enrolled in the Health Status Monitoring Program.  These fees are utilized to:

  • coordinate and ensure the successful scheduling of patients throughout the term of the enrolment
  • provide a Health Status Monitoring Report which is updated routinely

How much is a Physician Telemedicine Consultation?

All Telemedicine Consultations are covered by OHIP.  This fee includes Medication Delivery, access to a Licensed Physician, access to our support centre and a virtual record of all Telemedicine Consultations. If you do not have OHIP, most consultations are $50.
