Enrol, Schedule and Track
Enrol
Request to enrol clients by emailing enrol@conciergedoc.ca. CONCIERGEDOC will contact you to review all pertinent details and to confirm the enrollment.
Schedule
CONCIERGEDOC proceeds to contact and schedule your clients’ Telemedicine Consultation after which you will be notified.
Track
CONCIERGEDOC provides the referral source a Health Status Monitoring Report which is updated on an ongoing basis and includes all OHIP Consultation Notes.
When enrolling a client please provide the following information:
Client Name
Date of Birth
Date of Accident
Valid Contact Number
Valid OHIP Number with Version Code
Address
Brief Description
of injuries, issues, and or any additional information that may assist in the management of the patient
Representative Contact Information
All scheduled client appointments will be provided to the designated client representative on file.
